For shifts at Well Pharmacy, you need to complete, sign and send a number of forms to us. Here is the list of documents we need you to provide.
-DBS certificate reference number, issue date and your DBS update service reference number
-Copy of passport/work permit
-Indemnity Insurance proof
-Locum Starter Form fully completed and signed – download from here
-Locum Agreement signed and dated – download from here
-GDPR Declaration signed and dated – download from here
-A signed NMS Self-Accreditation form – download from here
-A photocopy of the front of your EPS Release 2 Smartcard – more information on this can be found in this booklet
-A copy of your CPPE Declaration of Competency for Discharge Medicines Service (DMS) Certificate
-A copy of Level 2 Safeguarding Status for Children and Vulnerable Adults dated within the last two years
-A copy of the email confirmation that you are a registered “Dementia Friend” – register here
-A copy of your Repeat Dispensing Certificate
Make sure you send all of the above in one email to hello@curisapp.com
We need these documents in place before you can be approved for any shifts with Well*. However, most other pharmacies require the same or similar documents so once this is done, you should be good to go.
